Works directly with patients, family members, physicians, and community providers to ensure collaborative and continuous care. Identifies and intervenes for suspected abuse, including sensitivity to age-specific concerns. Participates in multidisciplinary treatment team meetings, providing clinical input. Formulates discharge plans in collaboration with the multidisciplinary treatment team, the patient, and the family, ensuring outpatient appointments are scheduled upon inpatient discharge. Demonstrates working knowledge of the involuntary commitment process. Conducts comprehensive assessments to include risk assessment and establishes therapy treatment objectives.
