Advanced Practice Provider - Advanced Illness Management (AIM)

Key Responsibilities:

Essential Functions


The Physician Assistant / Nurse Practitioner will provide advanced palliative care to patients in their homes, helping them achieve the highest quality of life possible while managing complex medical needs.

  • Provide patient-centered palliative care to adults with advanced, life-limiting progressive and terminal illness.
  • Provide initial and ongoing clinical assessment for patients with advanced and/or progressive medical conditions.
  • Assess patients’ needs, in conjunction with patients’ wishes, and establish goals of care and recommend treatments and therapies that manage symptoms.
  • Collaborate with primary care provider, specialist, Population Health Transitional Care Management and Chronic Care Management, patients, and their families to effectively manage the palliative care needs of the patient; including emotional, spiritual, coordination of care, and bereavement support.
  • Facilitate referrals for additional care services when appropriate.
  • Prescribe medications and order diagnostic tests.
  • Maintain timely, accurate clinical documentation in compliance with professional standards.
  • Maintain safe and clean working environment by complying with procedures, rules, and regulations.
  • Protect patients and employees by adhering to infection-control policies and protocols.
  • Comply with federal, state, and local legal and professional requirements by studying existing and new legislation; anticipating future legislation; enforcing adherence to requirements; advising management on needed actions.
  • Maintain professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies.

Qualifications for Success:

Education, License/Certification, and Experience Requirements


Education: Master's degree from an accredited nurse practitioner / physician assistant program required.


Licensure, Certification, or Registration Required:

  • Current unencumbered license to practice as a Certified Nurse Practitioner (APRN) or Physician Assistant (PA-C) required.
  • Valid driver's license and current automobile insurance coverage.

Experience: Three years of experience as a Family Medicine NP / PA. Experience in a hospice or palliative care setting, preferred.

Work Setting: On-site and home visits


Job Type: Full-time


Schedule: Weekdays as determined by supervisor with shared on call coverage


Location: Corporate-Augusta, GA


Compensation: Market competitive base pay, commensurate with education and experience.
