Connects patients and families with resources for education, support, and clinical needs throughout their course of treatment. Works collaboratively with Patient Accounting and Coding to ensure compliance with patient charges and appropriate coding of procedures. Provides effective follow-up with structural heart patients per mandatory registry guidelines and ensures records are in EMR for registry data abstraction. Coordinates and hosts weekly multi-specialty physician team meetings, formulates agendas, and follow-up recommendations to facilitate patient care. Uploads required data to appropriate vendors and maintains communication with device representatives. Facilitates scheduling and authorization of procedures, both pre and post-procedure. Actively engages with Marketing to enhance referrals and provide community education. Other duties as assigned.
